Brochure Request
As part of the APWU Health Plan’s commitment to innovation the Health Plan will limit its production of 2013 federal brochures. With support from the Office of Personnel Management, the following changes will be made for Open Season Brochure distribution.
- All APWU Health Plan members will have the option of receiving a paper copy of the Plan brochure or an electronic version.
- Members can download an electronic version of the 2013 brochure by clicking here.
- For a paper copy of the 2013 brochure please fill out the form below.
- APWU Local Union offices will receive a reduced supply of informational flyers and brochures
- You may also choose to receive the Open Season Flyer which has information about the plan, rates and enrollment codes. Please click "Flyer" below.
