Effective January 15, 2022
An At-Home OTC test is a rapid antigen test that offers a quick and easy way to screen for COVID-19 at home.
The APWU Health Plan and UnitedHealthcare are covering those tests approved, cleared, or authorized by the FDA for use at home. Look for “FDA Emergency Use Authorization (EUA)” marked on the box.
COVID-19 At-Home OTC tests are NOT covered for routine testing related to employment. CDHP members may be required to sign and attest that the purchase of the OTC is not for employment purposes.
CDHP members will be able to obtain an at-home COVID-19 test at one of the preferred retailers through OptumRx with no upfront cost. In order for the purchase to be completed with no upfront cost, the OTC test must be purchased at the pharmacy counter of one of the preferred retailers.
Please note that if a CDHP member purchases an at-home COVID-19 test at any in-store or online retailer other than the pharmacy counter at one of the preferred retailers, they may also submit purchase receipt(s) for reimbursement at the CDHP member portal for a maximum reimbursement of $12 per test.
Online or in-store purchases completed on or after 1/15/2022 will be eligible for reimbursement.
To submit the reimbursement request online, follow these steps:
- Go to UnitedHealthcare
- Sign in to the member portal
- Select “Learn More”
- On the COVID-19 Resource Page, select “Start Reimbursement”
- Select “Start Request”
Members can also call the number on the back of their ID card and if requested, a customer advocate will mail them a form.
CDHP members do not need to obtain prior authorization or doctor’s orders to obtain a test.
Note: Many COVID-19 tests are sold as a two-pack. The two-pack test pack would be reimbursed at $24 ($12 for each individual test).
For CDHP members, the initial preferred OTC retailers for at-home COVID-19 tests are Walmart Pharmacy, Rite Aid Pharmacy, Bartell Drugs and Sam’s Club. Additional preferred retailers are expected to be added soon.

CDHP members may also purchase an FDA approved or authorized (EUA) at-home COVID-19 test at any in-store or online retailer other than one of the Preferred Retailers. This option will require the member to pay upfront. However, they may submit purchase receipt(s) with a purchase date of 1/15/2022 or after for reimbursement at the CDHP member portal for a maximum reimbursement of $12 per test.
